Does a step not work out? That is all right. Make it smaller, or choose another moment.

How it works
- Write down what you want to achieve, and why it matters to you.
- Think of a few small steps, five at most, and put them in the right order. For each step, write down when you will do it.
- Write down who can help you, and how you will notice that it is working.
- Print your plan, or save it as a PDF. Put it up somewhere you see it often.
- Have you done a step? Tick it, on screen or on paper.
How small is a small step?
So small that it almost happens by itself. ‘Move more’ is a goal. ‘Walk round the block for ten minutes tomorrow after breakfast’ is a step. Does a step still feel big? Then cut it into two smaller pieces.
Also write down when you will do the step. People who decide in advance when and where they will do something do it more often.
Does a step not work out? Look at what got in the way, make the step smaller or choose another moment.
On your own or together
You can make your plan on your own. You can also make it together with someone you trust, such as your care worker, a friend or someone in your family.
Are you a care worker? For each step, ask: ‘Can you do this, even on a not-so-good day?’ If the answer is no, make the step smaller together. Agree when you will look at the plan again.
Rather on paper
Take a sheet of paper and write down these questions, with space underneath:
- What do I want to achieve?
- Why does that matter to me?
- What are my small steps? Write down three to five, with a box in front and, for each step, when you will do it.
- Who can help me?
- How will I notice that it is working?
Your data
What you fill in is not saved and not sent. If you close or reload the page, everything is gone, including your ticks. So print your plan or save it as a PDF before you close the page.
Start again clears everything straight away, for example on a computer you share with others.
