How it works
The card is for a client who finds reading harder, or who quickly forgets an appointment. It shows what kind of appointment it is, when, where, with whom and what to bring. Each part has a picture and a word, in large letters. You see the card as it comes out of the printer: on a large screen while you fill it in, on a phone at the last step.
- Choose what kind of appointment it is: GP, neighbourhood team (wijkteam), dentist, council (gemeente), work, school, home visit, or a phone or video call. Each kind has its own picture.
- Fill in the date and the time. The card shows the date in words, for example ‘Tuesday 6 October’. Does your client need to leave on time? Then also fill in what time they need to leave.
- Write where the appointment is and with whom. Is someone coming to your client's home? Tick that, and the card will say ‘At your home’.
- Tick what your client needs to bring: proof of ID, health insurance card, the letter, medicines or phone.
- Check that the card is correct and press ‘Print’. The card is the size of a postcard (A6) and is printed on A4. Cut it out along the dotted line.
Good to know
- Go through the card together. Read it out loud and point to the pictures.
- Agree where the card will hang or lie, for example on the fridge or next to the keys.
- One card is one appointment. If your client has more, make a separate card for each appointment.
- You can print as soon as the kind of appointment, the date and the time are filled in. The rest can stay empty.
- The card is small. A long address or a long name sometimes does not fit. Then you will see a message and the card will not print. Make the address or the name shorter, or tick fewer things.
- No printer to hand? Choose ‘Save as PDF’ in the print window and print the card later.
By hand
Does filling it in not work? Then make the card yourself, on a small card or half an A5 sheet. Write large and clearly, one below the other:
- what kind of appointment it is, with a simple picture next to it;
- the day and the date in words, for example ‘Tuesday 6 October’;
- what time the appointment starts, and what time your client needs to leave;
- where it is, and with whom;
- what your client needs to bring.
