A little book about me

Choose a booklet

Three kinds, each with eight pages. Each page has sentence starters, stickers and a space to draw. You can change everything.

  • My user manual

A booklet in the language of a manual: how best to get on with me, and what you had better not do.

For people you spend a lot of time with: a new colleague, a support worker, the people you stay with.

Pages: How I work best · Warnings · Troubleshooting · Maintenance · Recharging my battery · Do not · Customer service

  • My story

Who you are, what is going on, what has been tried and what you want. So you do not have to keep telling it again.

For a new care worker, a new support worker or a first conversation.

Pages: Who I am · My people · What is going on · What has been tried · What helps · What I want · Agreements about this booklet

  • Passport for school or work

What you are good at, what you need help with and how someone can best talk to you.

For a new school, a work placement, a job or a new place.

Pages: This is me · What I am good at · What I need help with · How best to talk to me · This is when I am at my best · When things are not going well · Contact and agreements

Making it together: tips for care workers
  • You can do the writing if that is easier. Then read out what you write down and ask if it is right.
  • Do one or two pages at a time. Save the booklet as a file and carry on next time.
  • Does a sentence starter not fit? Ask what the other person would like to say here. A page can also stay empty: then it gets lines.
  • Feel free to ask what a drawing means, but do not write anything next to it without asking.
  • Do not make a copy for the client file without asking.
  • You can cut and fold together. The folding instructions are on the second sheet.

How to fold your booklet

E-powerment

  1. 1Print the sheet

Print on A4, landscape. When printing, choose actual size or 100 per cent. The top row is upside down: that is how it should be.

  2. 2Fold along the lines

Fold the sheet back and forth along every dotted line, then unfold it again. That makes the folding easier later.

  3. 3Cut in the middle

Cut along the dashed line in the middle. It only runs across the two middle boxes, not to the edge.

  4. 4Fold it in half

Fold the sheet in half lengthways, with the pages on the outside. The row that was upside down goes to the back.

  5. 5Push the sides together

Hold the two short sides and push them towards the middle. The cut opens up and forms a plus sign.

  6. 6Fold the booklet shut

Fold all the pages flat around each other, with the front on top. Your booklet is ready.

How it works

  1. Choose a booklet: My user manual, My story or Passport for school or work.
  2. Fill in the pages one by one. Choose a sentence starter and finish the sentence, or write something of your own. Anything you leave empty gets lines, so you can fill it in by hand later.
  3. Add stickers or make a drawing. On the front you put your name and, if you like, a self-portrait.
  4. Leaf through your booklet to see how it will look.
  5. Print it on one sheet of A4. Cut once and fold it into a booklet. Would you rather not fold? Then just print it, two pages per sheet.

Why a booklet

With a new care worker, a new school, a new job or a place to stay, you often tell your story all over again. A small booklet does that for you. It is more personal than a form, and it fits in your coat pocket.

Three kinds of booklet

  • My user manual uses the language of a manual: how I work best, warnings, troubleshooting, maintenance, recharging my battery and do not. For people you spend a lot of time with.
  • My story is for a new care worker: who you are, who is around you, what is going on, what has been tried, what helps and what you want.
  • Passport for school or work is about what you are good at, what you need help with and how someone can best talk to you.

You can change every title and every sentence.

Making it together

Tips for care workers who make the booklet together with a client are in the tool under Making it together.

Cutting and folding

The booklet fits on one sheet of A4. Print it at actual size, in landscape. The top row is upside down: that is how it should be. You only cut along the dashed line in the middle. The folding instructions are in the tool step by step, and on paper on a second sheet. This way of folding is called a zine.

Good to know

  • Everything works with the keyboard. You move stickers with the arrow keys, or with the buttons below the picture.
  • You can draw with a mouse, finger or pen. With the keyboard you can add stickers or write instead.
  • Every page has a text version. Under View, choose Read the booklet as text.
  • Are you writing about things that are hard, and do you want to talk about them? Do that with someone you trust, your GP or a care worker. In acute danger, call 112. If you are thinking about suicide, call 113 (the Dutch suicide prevention line) or 0800-0113 for free.

    River of life
